Two employees were lowering a 6-foot, 8-inch section of pipe to the ground with a come-along. The pipe slipped out of the rigging and struck a 6-foot stepladder one of the employees was standing on. The employee fell to the ground, suffered a skull fracture, and was hospitalized.

An employee was operating a CNC machine that had a chip conveyor extending from it. The employee turned the CNC machine off to retrieve a chip ball piece of scrap from the conveyor. As he was retrieving the scrap, the machine was unintentionally activated. The employee's glove became caught in the moving parts and his fingers were caught where the conveyor meets the CNC machine. His left-hand index and middle fingers were partially amputated.
